I look forward to supporting your health! To schedule your visit, please use the intake assistant in the bottom right hand corner. It will guide you through the initial steps of the intake and scheduling process, and then once you have access to your patient portal you can go there to finish filling out the office policy and new patient forms. But first, there are two important things to note before starting:

1.  When entering dates, you can select the date from the calendar function, but will need to click the “next” arrow in the lower right corner in order to continue once you have entered your date of birth (day, month, and year)

2. When proceeding to payment, make sure to scroll to the top of the page. Otherwise it will look like a blank screen, but the information box is simply at the very top of the page. We do require a $100 deposit with a card on file in order to hold your new patient appointment time slot.
