To schedule an appointment with Dr. Holtski, or change the time of an existing appointment, please use the virtual intake assistant in the bottom right hand corner. This will guide you through the scheduling process, and then once you have access to your patient portal you can go there to finish filling out the intake forms and member agreement form.

Depending on the extent of your health history, the whole process may take 20-30 minutes to complete. Please note that for every appointment, we require a $100 deposit at the time of booking that goes towards the cost of your visit. The virtual assistant will ask a few questions to walk you through:
Scheduling your visit
Saving your payment information
Then, once you are registered as a patient, you will receive an email to log onto your patient portal. Make sure you are able to log on and complete the following:
Signing our office policy forms
Completing your health history form
Completing a brief depression and anxiety screening questionnaire
Please note that all three of these forms will need to be completed prior to your initial visit. Typically, it is best to fill them out right now at the time of booking. Your thoroughness in answering the questions helps me to better prepare for our first visit together.
